1. Please review the Tattoo FAQ prior to your appointment.

2. A Release Form must be signed at time of procedure. If you have any doubts about any portion of the procedure, please contact Leah@leahink.com.

3. Cosmetic tattoos are real tattoos, implanted in skin with needles. During your first appointment, we will discuss expectations, steps in the procedure, and after-care.

4. Please request a patch test if you are or might be allergic to latex, topical anesthetics, cosmetics, whitch-hazel, etc. Consult your physician if you are unsure. Clients assume 100% of the risk involving reactions to anesthetics, latex or other materials used during the procedure and after-care. LeahInk shall not be held responsible for undesireable results or adverse or allergic reactions.

5. Leah ink salon hours are available by confirmed appointment only. Please contact Leah with any questions.

6. All clients are required to have before and after photos of the procedure, and photos 4-6 weeks post procedure for legal & marketing purposes. Clients may opt out of marketing materials.

7. Clients hereby agree to enter property and premisis at their own risk. Leah Ink shall not be responsible for any injuries, damages or loss to persons or personal property sustained as a result of ingress, egress or while on the property.

8. The Leahink Salon and property is 100% non-smoking.
